Please access the correct application for the account type and fill out completely. Be sure to include all documentation as outlined in the Documentation Matrix found in the application. Failure to include all documentation and to fill out and sign all applicable areas will result in delays in account opening.

Please Note: We can now accept faxed applications. Applications may be faxed directly to SEI at 610-676-1021 for processing.

We have updated our SBLOC applications to make them more user friendly and to allow you and your client to choose which features fit best. Take notice that we have removed our combination applications because of this update along with the lending addendums and standalone SBLOC application. The new SBLOC applications may be used when adding an SBLOC to an existing or new CAA or when opening just an SBLOC with no CAA.    

Instructions for completing applications:

1. Fill out an application below or from SEI’s Business Builder in the Forms Library
2. Print the completed application and have both you and your clients sign where indicated 
3. Fax or Mail the signed paper application to SEI for further processing
